Based on a nursing home's performance on 3 sources: health inspections, staffing and quality measures.
Original Medicare typically covers short-term rehabilitation for up to 100 days (lifetime limit) following a hospital stay of at least three inpatient days or under specific qualifying circumstances, once your deductible is met. For long-term nursing home care, Medicaid may provide coverage after exhausting long-term care insurance and personal savings, depending on the facility. It's important to verify payment options and costs directly with the nursing home to ensure clarity and avoid unexpected expenses.

My grandmother has been a resident at Boulder Creek for several months now, and we've been so impressed with the care she's received. The staff is truly dedicated and attentive, and they've made her feel comfortable and well-cared for. The facility itself is clean and well-maintained, and the activities they offer keep her engaged and active.

I was hesitant about placing my father in a skilled nursing facility, but I'm so glad I chose Boulder Creek. The staff is incredibly kind and compassionate, and they go above and beyond to meet my father's needs. The therapy services are excellent, and he's made significant progress since he arrived. I highly recommend this facility to anyone looking for quality care for their loved ones.

The staff at Boulder Creek is generally nice, but there have been times when communication could be better. There have also been some issues with the food quality, but overall it's a decent facility. I think there is room for improvement, but it's not a bad place.
