Kaiser Permanente Post-Acute Care Centers provide a comprehensive range of services designed to aid individuals in their recovery and transition back to their homes or preferred living environments. These centers offer a structured and supportive environment that promotes healing, rehabilitation, and overall well-being. The experienced staff, comprised of skilled nurses, therapists, and other healthcare professionals, work collaboratively to address individual needs and develop personalized care plans. The facilities are equipped with state-of-the-art technology and amenities, ensuring optimal comfort and recovery. Upon admission, a comprehensive assessment is conducted to identify each patient's unique requirements and goals. The care team then works diligently to establish a personalized plan that includes medical management, physical therapy, occupational therapy, speech therapy, and other specialized services. The centers also prioritize social and emotional well-being by offering recreational activities, social interaction opportunities, and support groups. The goal of Kaiser Permanente Post-Acute Care Centers is to provide a seamless transition from hospital care to home, fostering independence and maximizing each individual's potential. These centers strive to create a comfortable and supportive environment that encourages progress and empowers patients to regain their strength and reclaim their lives....
Based on a nursing home's performance on 3 sources: health inspections, staffing and quality measures.
Original Medicare typically covers short-term rehabilitation for up to 100 days (lifetime limit) following a hospital stay of at least three inpatient days or under specific qualifying circumstances, once your deductible is met. For long-term nursing home care, Medicaid may provide coverage after exhausting long-term care insurance and personal savings, depending on the facility. It's important to verify payment options and costs directly with the nursing home to ensure clarity and avoid unexpected expenses.

This facility exceeded my expectations. The therapists were highly skilled and helped me regain my strength after my surgery. I highly recommend this center to anyone looking for post-acute care.

While the care was good, the food could be improved. There wasn't much variety and it wasn't always appetizing. However, the staff were very responsive to requests for changes.
