Based on a nursing home's performance on 3 sources: health inspections, staffing and quality measures.
Original Medicare typically covers short-term rehabilitation for up to 100 days (lifetime limit) following a hospital stay of at least three inpatient days or under specific qualifying circumstances, once your deductible is met. For long-term nursing home care, Medicaid may provide coverage after exhausting long-term care insurance and personal savings, depending on the facility. It's important to verify payment options and costs directly with the nursing home to ensure clarity and avoid unexpected expenses.

My mother has been at Ascension Oaks for a few months now and we've been very pleased with the care she's received. The staff is friendly and attentive, and the facility is always clean and well-maintained. They offer a variety of activities and programs to keep residents engaged, which is great. I would definitely recommend this facility to others.

My father was recovering from a stroke at Ascension Oaks and the rehab program was fantastic. The therapists were incredibly skilled and patient, and they helped him regain his strength and mobility. He made great progress and was able to return home sooner than expected. The staff was very kind and supportive throughout his stay.

While I'm happy with the care my grandmother is receiving at Ascension Oaks, there have been a few issues with communication and responsiveness from the staff. It's been a bit frustrating trying to get updates on her condition at times, and I've had to follow up several times. However, the nursing staff is good and she seems happy overall.
