Based on a nursing home's performance on 3 sources: health inspections, staffing and quality measures.
Original Medicare typically covers short-term rehabilitation for up to 100 days (lifetime limit) following a hospital stay of at least three inpatient days or under specific qualifying circumstances, once your deductible is met. For long-term nursing home care, Medicaid may provide coverage after exhausting long-term care insurance and personal savings, depending on the facility. It's important to verify payment options and costs directly with the nursing home to ensure clarity and avoid unexpected expenses.

My grandmother received excellent care at Azalea. The staff was always kind and attentive, and the facility was clean and well-maintained. I highly recommend this center for anyone looking for skilled nursing care.

I was very impressed with the level of care my father received at Azalea. The therapists were highly skilled and helped him regain his mobility. The facility also provided a variety of activities and social programs that kept him engaged and happy.

The care at Azalea was decent, but I wish there were more opportunities for socialization. My mother sometimes felt isolated, especially during the evenings. Overall, it was a good experience, but there is room for improvement.
