Based on a nursing home's performance on 3 sources: health inspections, staffing and quality measures.
Original Medicare typically covers short-term rehabilitation for up to 100 days (lifetime limit) following a hospital stay of at least three inpatient days or under specific qualifying circumstances, once your deductible is met. For long-term nursing home care, Medicaid may provide coverage after exhausting long-term care insurance and personal savings, depending on the facility. It's important to verify payment options and costs directly with the nursing home to ensure clarity and avoid unexpected expenses.

The staff at Keizer Nursing and Rehabilitation are so kind and caring. My grandmother was there for a few months after a fall and they helped her regain her strength and mobility. They always listened to our concerns and kept us informed of her progress. I would highly recommend this facility.

The facility itself is clean and comfortable, but the food could use some improvement. The staff were helpful, but sometimes seemed short-staffed. Overall, it was a decent experience but I think there's room for improvement.

I was so impressed with the level of care my mother received at Keizer Nursing and Rehabilitation. The therapists were amazing and helped her get back on her feet after her stroke. The staff was always attentive and made sure she was comfortable and well-cared for. I couldn't have asked for a better place for her to recover.
