Based on a nursing home's performance on 3 sources: health inspections, staffing and quality measures.
Original Medicare typically covers short-term rehabilitation for up to 100 days (lifetime limit) following a hospital stay of at least three inpatient days or under specific qualifying circumstances, once your deductible is met. For long-term nursing home care, Medicaid may provide coverage after exhausting long-term care insurance and personal savings, depending on the facility. It's important to verify payment options and costs directly with the nursing home to ensure clarity and avoid unexpected expenses.

My grandmother stayed at Five Points for a few weeks after her surgery. The staff was so kind and patient with her. They kept her comfortable and helped her get back on her feet. I would definitely recommend this facility.

I was impressed with the level of care at Five Points. The nurses and aides were very attentive and responsive to my mother's needs. The facility was clean and well-maintained, and the food was decent. My mother was happy there.

Five Points was okay. The staff was friendly, but I felt like the communication could have been better. I had some concerns about the level of care my father was receiving, but they were addressed eventually.
