Based on a nursing home's performance on 3 sources: health inspections, staffing and quality measures.
Original Medicare typically covers short-term rehabilitation for up to 100 days (lifetime limit) following a hospital stay of at least three inpatient days or under specific qualifying circumstances, once your deductible is met. For long-term nursing home care, Medicaid may provide coverage after exhausting long-term care insurance and personal savings, depending on the facility. It's important to verify payment options and costs directly with the nursing home to ensure clarity and avoid unexpected expenses.

The staff at Westward Trails are truly amazing. They are so caring and attentive to my grandmother's needs. She has received excellent care and the facility itself is clean and comfortable. I would highly recommend this facility to anyone looking for quality nursing care.

While the staff is friendly and helpful, there have been a few instances where communication could have been better. Overall, it's a decent facility, but there is room for improvement.

My father has been at Westward Trails for several months now and we are very happy with the care he has received. The therapists are excellent and the facility is very clean and well-maintained. The food is also very good!
